With over 25 years of continued practice in your community, you will have an experienced team of professionals who will guide you on your personal path to healing and recovery as well as help you navigate the complex insurance and other financial questions and concerns you may have.

You, your physician or a family member can start this process and request an appointment at any of our locations. Expedite this process by having the following information available at the time of your call:
- Name and phone number of your referring physician (if applicable)
- Up-to-date insurance card and related information

Once your appointment has been confirmed please download and complete the following document:
If you are unable to download this information let our scheduler know and we will send you the package in the mail or by email.
Our staff can also help you complete the forms at the time of your registration prior to the appointment. Please arrive at least 15 minutes prior to your appointment to complete your check in.
Remember to bring ALL your medication bottles or packages with you to the visit, so we can accurately confirm and update your medication list.
